The Lafayette Public Library works with the IRS to increase the availability of federal tax forms and publications by providing free tax products for the public at all library branches. The IRS is only sending LPL the following forms: 1040, 1040 A, and 1040 EZ. They are not sending instruction books, schedules, or any other forms. The library will have one copy of reproducible forms that patrons can photocopy (10 cents per page). To request paper copies to be mailed to you, go online to IRS.gov/orderforms. To download forms: IRS.gov/Forms; or call: 1-800-829-3676 The IRS will begin processing paper and online forms on January 20, 2015.

The Louisiana Department of Revenue no longer provides printed tax booklets to individuals; forms for full-time residents of Louisiana are available online by visiting the following website: http://www.rev.state.la.us/sections/taxforms/default.aspx. In order to request a form to be mailed to you, call the Louisiana Department of Revenue at 1-888-829-3071 (toll-free) or by visiting the above Louisiana Department of Revenue website.
